Grande Prairie Home Care

 

Provides medical support for people so they can live in their own homes

Home Care helps people who live at home and need support such as; long-term care, palliative care and respite services.

A case manager, who is usually a nurse, will meet with the clients and help decide what services will best support them.

Queen Elizabeth II Hospital

The hospital has a palliative unit with ten beds. Palliative care services include the management of pain and other symptoms as well as social, emotional, spiritual, and practical support.

Telehealth Pain and Symptom Control Clinic-Edmonton: Cross Cancer Institute

Clinic for patients in rural communities having problems with pain or other cancer symptoms who do not have access to specialists locally.

This clinic helps patients with the pain and symptoms associated with cancer by providing real-time support to physicians caring for cancer patients in rural and remote communities.

High-definition televisions link the remote care centre to a multi-disciplinary team at the Cross Cancer Institute in Edmonton. The team can then assess the patient without the patient leaving his or her home community.

Volunteer Resources – Grande Prairie: Queen Elizabeth II Hospital

The Volunteer Resource Coordinator can coordinate a volunteer for palliative support for patients and families in the Grande Prairie healthcare facilities and in their homes upon referrals from nursing and/or families (e.g. QE II Hospital, Home Care, and Public Health).
